Provides case management services to all adult clients, regardless of age, who are receiving community-based services in their home, funded through Title XIX Medicaid and state-funded chore personal care. Activities include conducting assessments and re assessments, authorizing and reauthorizing services, assisting clients with the Medicaid eligibility review process, locating and hiring providers, arranging and preparing for fair hearings, as well as regular, ongoing case management activities.
Aging and Long Term Care helps older adults and adults with disabilities live at home independently through in-home care and case management services.
Offers Case Management (CM) to assist older adults and adults living with disabilities set up a safe plan of care to maintain independence. Persons on CM receive help from a caregiver (for help with personal tasks such as bathing, transportation, light housekeeping, dressing, and more), receive annual assessments to determine the best pathway of care, and have access to other programs to help them remain safe and happy at home.
Upon referral, a RN Consultant can also provide medical assessment, educate caregivers on client specific information, consults and coordinates with the client care's team (such as physicians, social workers, and others), and evaluates clients skin conditions, nutrition, hydration, disease process, response and tolerance of medication and more.

Coordinates a low-cost, comprehensive system of health promotion and disease management programs, including the Enhance Fitness Program, which offers group exercise for older adults. The program is designed to improve older adults' physical conditioning level and decrease risk of falling. Classes are held three times a week and each hour-long class includes strength training, cardiovascular conditioning, and balance and stretching exercises. Enhance Fitness Program is a program of Senior Wellness Project of Sound Generations and is offered primarily at senior centers throughout King County.
WOW - Working on Wellness offered at Dungeness Valley Health & Wellness Clinic - Sequim Free Clinic
Offers free on-site educational resources and community outreach programs at Trinity United Methodist Church with guest presenters covering a variety of health topics.
Provides on-site educational resources related to health and wellness to empower clients to become masters of their own state of health. In particular, the clinic provides information and tools to help clients prevent illness and disease, and to help those with persistent illness to better manage the disease process to enhance quality of life. Reading materials and educational videos on topics related to health maintenance are available at the Clinic. Also offers a free community outreach program that delivers health and wellness information from guest presenters to the whole community for disease prevention and self-management of chronic illness.
