Delivers hospice services in homes, nursing homes, adult family homes, assisted living facilities, the Hospice Center and contracted hospitals.  Services include intermittent nursing care, 24-hour on-call support services, social services, hospice aides, trained volunteers, spiritual counseling and bereavement services.   Acute inpatient hospice care is available at the Evergreen Hospice Center and several contracted hospitals.   Respite care is available for caregivers at contracted nursing homes.

Provides adult day care for older adults and respite for caregivers.  Activities include social interaction, word games, reminiscing, intellectual activities, music, exercise sessions, hot lunch and a morning snack.

Provides temporary respite care to parents of people with development disabilities.

Offering supportive independence with exceptional in-home care. Services include post-hospitalization recovery support, medication reminders, personal care and hygiene assistance, transportation, meal preparation, errands and grocery shopping, light housekeeping, help with laundry, memory support and companionship.

Provides home health care for those recovering from surgery or illnesses in the comfort of their own home through skilled nursing, physical therapy, and other care services. Also provides home medical supplies for patients or customers, such as oxygen equipment; home ventilators; nebulizers; canes, crutches, and walkers; wheelchairs and accessories; orthopedic braces; hospital beds; bedside commodes; pressure reducing mattresses/overlays; pediatric products; enteral nutrition; ostomy supplies; aids to daily living; and bath safety equipment.

Helps prevent or delay the need for institutional care for people with HIV/AIDS by helping them maintain independence at home, and by providing daytime relief for their families or caregivers through an outpatient program. Services include: -  Health monitoring -  Medication adherence -  Physical and occupational therapy -  Recreational and social activities -  Counseling and support groups -  Hot meals - Hair Cuts - Laundry/Showers -  Nutritional counseling   Interpreter service available Clients are provided with disability bus passes when eligible.

Adult Day Center Offers individualized care, case management and recreation activities, including social events, crafts, music, water therapy and horticulture programs. The W.I.S.D.O.M. program offers a social day program for individuals experiencing early memory loss. Weekly participation preferred. Short-Term Respite Gives quick, easy respite to families and caregivers who need it for limited periods of time. Provides transportation to and from the center for those who need it.

Kitsap Applied Technologies offers a facilities-based respite program that provides services designed to promote skill acquisition and social interaction. We believe Respite can be more than just recreation. KAT’s Respite Program provides temporary direct care and supervision in a safe facility-based environment. The service is designed to help meet the needs of the primary caregiver as well as the served individual. Program activates include: Communication Skills Building; Social Skills Building; Physical Exercise; and Multi-Media Appreciation. Our services are designed to be individualistic and keyed to the particular needs of those served. We provide respite services to individuals twice a week from 12:30 p.m. to 3:30 p.m. on Mondays and Wednesdays. Our Respite program provides a brief span of free time for the care givers to spend some quality time alone to recharge their "care-giving batteries" which run low. The program offers educational opportunities, physical exercise, and socialization skills to the participants. The program emphasis social interaction which helps participants develop their interpersonal skills.

Supports individuals with developmental disabilities by providing community living services to help them live in their own homes within their own communities. Individualized teaching and training may include:   -  Personal hygiene -  Cooking, cleaning, and laundry -  Grocery shopping and menu planning -  Paying bills and budgeting -  Assistance with medications -  Doctor appointments and following doctor orders - Support with communication devices, transportation, and support with activities

Provides enrichment for people who benefit from supervision but who require minimal assistance with most activities of daily living.   Services include but not limited to: - Health services for people who require medical monitoring   - Assistance with many activities of daily living - Daily sessions for socializing, movement, and art activities - Access to a fully accessible Comcast Lift Zone computer lab - Digital Navigators Technology Classes available - Family caregiver support, including free training, counseling and other support services for family caregivers

Provides a range of services for elders, from companionship programs and volunteer services that help with day-to-day chores to relief for overburdened caregivers. Catholic Charities provides both help and connection. Volunteer Services: provides basic in-home assistance to low-income elders and adults with disabilities to help them continue living independently, safely, and with dignity in their own homes. Only local rides; no work related transportation. Volunteers can provide services such as: local transportation for appointments and errands (no long distance), yard work, light housekeeping and laundry, snow shoveling, minor home repairs, wheelchair ramps (not an immediate service; if qualifications aren't met, might have to pay material out of pocket), and wood provision. Senior Companion Program: serves one-on one with the frail elderly and other homebound persons who have difficulty completing everyday tasks. Provides companionship as they assist with grocery shopping, transportation to medical appointments, and other tasks. Volunteers are individuals over 55 years of age with a limited income, they receive a small stipend for an average of 15-20 hours of service per week. Respite Adult Day Center in Yakima: gives frail or cognitively disabled adults the opportunity to make new friends and participate in a host of enjoyable activities in a safe and comfortable environment. A team of staff and volunteers are present to provide personalized support, encouragement and assistance to each participant. Provides some down-time to those who provide care to disabled individuals. The disabled individuals must have a memory loss situation (such as Alzheimer's, stroke, MS etc.) for the caregiver to be eligible for this program. This program works in conjunction with Aging and Long Term Care but is overseen by Catholic Charities Serving Central Washington.
