Contact agency for intake appointment, referral required if on Medicaid.
Eligibility
Must be 18 or older, in need of supervision or monitoring of a health condition, and in need of maintenance.
Fee
Accept Apple Health (Medicaid), Long Term Care Insurance, Private Pay, and offers a sliding fee scale. Medicaid is accepted only if patient is referred by a case manager.
