Home Health Home Care Services offered by MultiCare Yakima Memorial at Home Health, Hospice, and Transitions
Provides in home care for those who are recuperating from an accident or surgery. Also available is the Link Life service which is a personal alert device.
Home health is a program for those who need nursing care, rehabilitation or therapy at home. The Home Health team of registered or licensed nurses; home care aides; and occupational, speech or physical therapists work closely with your physician. For more information, call 509-574-3606.
The Home Health team
Registered/licensed nurses provide direct, skilled nursing care according to the plan of care developed with the patient's physician.
Home care aides assist with personal care, such as bathing, grooming and routine exercises.
Occupational therapists instruct patients in techniques to relearn the tasks of daily living, such as dressing, cooking, eating and bathing, as well as energy conservation and use of assistive devices.
Physical therapists assist with the patient's improvement of mobility, fall prevention and safety, ultimately reducing pain and helping the patient regain his or her maximum use of physical skills while recovering from an illness, injury or disability.
Speech therapists assist patients in improving communication challenges, such as difficulty understanding other people, redeveloping the ability to speak clearly or at all, and literacy-related issues. The speech therapist may also help with difficulties in eating or swallowing.
Social workers assist with community referrals, caregiver issues and financial concerns. They can also assist with connecting patients to an emergency response using a system that provides patient safety and security via phone technology.

Ashley House provides a "home away from home" environment for the children and their families while delivering complex medical care and family support.
Children and families receive services individualized to meet their needs. Ashley House provides or arranges for:
- Twenty-four hour nursing care in a home-like setting. All care and treatment is under a doctor’s orders.
- Social work services and family support.
- Transportation to and from Doctor’s appointments.
