Our Rural Health Clinics provide a full range of high-quality, comprehensive family health care.  Offers services for primary care, general surgery, colorectal surgery, sports medicine, orthopedics, and women's health. The three certified Rural Health Clinics are departments of Forks Community Hospital. Two RHCs are located in Forks, Washington, and one is located in Clallam Bay, Washington.

Provides pregnancy and parenting support programs. Services include pregnancy support, options and birth control counseling, family planning, doula services, breastfeeding support, post-partum depression, post-abortion and miscarriage help, and parenting help.

Provides maternity case management, maternity support services, First Steps Program, assistance with breastfeeding, and childbirth education classes. Other services and programs include: -  Breastfeeding support -  Car seat safety check -  Childbirth Education Classes -  Children with Special Needs -  Diet and nutrition counseling -  Family Planning services -  Free pregnancy testing -  Help with community resources and insurance information -  Infant Toddler Program -  Information and education about baby care and parenting skills -  Information and education about pregnancy, labor and delivery -  Outreach/Promoters Program -  Parent to Parent -  Substance abuse counseling -  Assistance in accessing dental and vision care -  Assistance in applying for Medicaid services -  Assistance in obtaining early and regular medical care -  Assistance in obtaining paternity information -  Assistance with housing and transportation

Offers specialized care for women. Services include: - comprehensive women's health care, - gynecology, - obstetrics, and - free pregnancy tests. Maternity Care - Provides routine and high-risk pregnancy care - Includes prenatal care and delivery at a local hospital. Maternity Support Services - MSS/Infant Case Management - ICM Households using Apple Health may also qualify for: - Referrals to community programs and services - Pregnancy and infant care education - Assistance with finding housing, employment, clothing, furniture, transportation - Assistance with finding financial assistance. - for those eligible, support services continue until the baby's first birthday. May also provide information on Safe Sleep and access to cribs to those receiving ICM services.

Gives pre-natal vitamins to women who need them.

Specializes in primary care family medicine, including - Preventive health exams - Prenatal care - Acute care visits - Minor procedures - Health education - Follow-up care from hospital visits

Provides support and information to pregnant women who are also on or eligible for Apple Health (Medicaid). Nurses, social workers, and community health workers support expectant mothers before birth and for two months after the birth of the child. Services are all covered by Medicaid, and may include supports like: - prenatal and postpartum education groups - breastfeeding groups - Doula services - transportation assistance during the pregnancy - Hospital tours - Behavioral Health or Chemical Dependency Services - Dental Care and - enrolling in WIC. May also provide information on Safe Sleep and access to cribs. Depending on the needs on the needs of the family, they may also receive Infant Case Management which provides support and guidance until the baby is one-year old. Purpose of the program is to help women have a health pregnancy outcome.

Offers primary health care for the entire family, including OB and pediatric services at multiple sites. Some locations offer Ear, Nose, and Throat services.

The Center offers general outpatient clinic services. Provides general comprehensive medical and dental, mental health and substance abuse counseling, WIC, family planning, community health outreach (CHR) and health education.

Provides obstetric care, including prenatal through postpartum medical care. First Steps First Steps helps income-qualified pregnant women or new mothers connect to health care that includes nutrition-based services. Ensures mother and baby are healthy through pregnancy, childbirth and the first year of life by providing - Prenatal care - Medical, vision, and dental care - Delivery - Post pregnancy follow-up - One year of family planning services post pregnancy - One year of full medical for newborns

Provides a wide range of community health services in selected sites throughout Washington, which include: Maternity Support, Infant Case Management, Maternal Child Health, Maternal Child Health, & Maternity Case Management.   Maternity Support Services: Enables healthcare providers to educate and support women and their family during pregnancy and after delivery.   Services include; - Community Health Nurse - Behavioral Health Specialist - Case Manager - Registered Dietician Provides personalized home and office visits focusing on your needs and concerns at certain stages of your pregnancy and after delivery. For example women may receive education on Labor and Delivery, counseling services to help cope with the life changing circumstances that come with a new pregnancy, breastfeeding, and nutrition education. *  Services are offered in: Grandview, Prosser, Spokane, Toppenish, Walla Walla, Wapato, and Yakima.   Infant Case Management: Enables healthcare providers to educate and support women and their family after the baby delivery. Services are provided by a member of the agency's interdisciplinary team. The goal is to improve the parent(s) self-sufficiency in gaining access to needed medical, social, educational, and other services. *  Services are offered in: Grandview, Prosser, Spokane, Toppenish, Walla Walla, Wapato, and Yakima.   Maternal Child Health: Enables healthcare providers to address healthcare needs and concerns regarding the child. Offers comprehensive obstetric care for pregnant women. Provides pre-delivery services, including regular physical exams and lab tests, prenatal screening and referral, and self-care. Other services include post-partum follow-up services. Services provided by family medicine doctors.

Provides adult medical care including yearly exams for all adults, and immunizations such as COVID-19, and flu vaccines. Other services provided include behavioral health evaluations, routine health exams, colorectal and prostate cancer detection, newborn/infant screenings, pediatric care, and extensive sports physicals. Additionally, provides care customized to the unique needs of older adults, including Medicare annual wellness visits.

Provides preventative health services to help people have a healthy pregnancy.   Offers family-centered obstetrical care, including delivery at Swedish - First Hill. Provides individualized education and counseling services for women and their partners before and after birth.   Offers the Parental Support Services case management program for low-income people who receive state medical assistance.   Team includes a nurse, nutritionist and a social worker.

This department functions as a health clinic, focusing on medical diagnosis and appropriate treatment and/or referral.

Provides support and information to pregnant women related to prenatal and postpartum health.   Services that may be available: - Prenatal and postpartum education groups - Breastfeeding groups - Transportation assistance during pregnancy - Hospital tours in English and Spanish - Behavioral health counseling - Dental care Additionally provides support and guidance in the care of the infant from 2-3 months old through his or her first birthday for those who need additional help.

Provides comprehensive pregnancy-related services and ongoing obstetric care.   Offers specialized care for pregnant women in areas such as the following: -  Diabetic care in pregnancy -  Premature birth prevention -  Care for multiple gestations (twins, triplets) -  Care for complex medical conditions during pregnancy -  Prenatal genetic counseling -  Pre-implantation genetic diagnosis -  Fetal diagnostic testing

Provides holistic pregnancy (prenatal) care, delivery and postpartum care (after baby is born) through certified nurse-midwives who have gone through advanced training to care for patients across their life span.   Midwives attempt to reduce Cesarean-section rates, traumatic deliveries and unneeded medical intervention.   Connects patients with community resources, and helps them understand their health care options.   Deliveries are at Swedish First Hill Birth Center.
