Specially trained Community Health Nurses visit families in their home throughout the pregnancy and continue to visit until the child is 2 years old. The nurses visit weekly and bi-weekly during the pregnancy, infancy and toddler period. The education curriculum includes information during each phase of the development. This relationship based model builds on the parent’s strengths supporting the parent child relationship, healthy parent-child outcomes and goals for the future helping first time parents succeed. This program is free to eligible women.

Offers free nurse home visits for caregivers and their newborns. Provides a medical check-up on the birth parent and the baby, as well as education for the family, lactation assistance, answers to any questions the family may have, and connection to local resources that help families thrive. Also offers weekly support with a virtual and in-person Baby Lounge - a place for all families with babies to connect and learn together as parents. Groups are facilitated by a health professional. There are child care stipends available, and refreshments are provided.

Provides case management services from the baby's 3rd month to one year. Services include resources for daycare, housing, financial assistance, transportation, medical, dental, counseling, employment, education, advocacy, legal issues, WIC and parenting classes. Also includes referral, linkage, and warm hand-off to many community agencies and resources. May also provide information on Safe Sleep and access to cribs to those receiving services.

Offers an evidence‐based home visitation case management for pregnant and parenting women with substance use disorders. - Helps clients access alcohol, drug, and mental health treatment; - Assures that children are in safe, stable home environments and receiving appropriate health care; to - Links mothers to community resources that will help them build and maintain healthy, independent family lives; and to - Prevents future births of children exposed prenatally to alcohol and drugs. Pregnant and parenting women are enrolled for three years. Clients are not asked to leave the program if they relapse or experience setbacks.
