Delivers healthy, home-cooked meals to participants' homes within 24 hours of discharge from the hospital. Program also includes wellness check phone calls over 30 days from the first meal delivery. Must live in South King County.

Assists in arranging needed services, financial assistance, and helps counsel patients and family.
