Provides home health care coordination of medical, behavioral health, long-term services and other community support for eligible Medicaid clients (who also receive Medicare) at no cost.
Provides home health care coordination of medical, behavioral health, long-term services and other community support for eligible Medicaid clients (who also receive Medicare) at no cost.
Service is provided in order to partner with clients, their families, and service providers to ensure coordination across systems. Additionally, a person-centered health action plan is developed with each client to improve their quality of life and slow the progression of chronic diseases; focus is on self-management.

Ombudsmen protect and promote Resident Rights guaranteed under federal and state law and regulations. Long-Term Care Ombudsman are trained to receive complaints and resolve problems in situations involving quality of care, use of restraints, transfer and discharge, abuse and other aspects of resident dignity and rights.

Provides a free intake to find the right care solutions. Has nurses, care managers and Placement Navigators to help families make the right decision regarding long term care, home care and other needs. Also has an instructional page for Medicaid.

Placements may include meeting with a Senior Living Adviser who would help families tour facilities, screen potential home care providers, and provide information on caregiver coping.
Helps families determine where is the best skilled nursing home or rehabilitation facility for their needs.
Helps families identify supported living options or in-home care services, and helps coordinate services.
Home Care Coordination services helps families understand which type of care is appropriate based on their unique needs; such as aging, chronic illness, and disabilities.
The analysis includes a projected cost for care and what services would be beneficial for long term care goals.
Also offers a free care planning support service to help a family identify and agree on the required care needs, and each family remember's role in meeting those needs.
Care Management provides an evaluation of one's ability to function independently.
Care managers can provide medication and treatment management, arrange for any necessary home modification, recommend and manage support services like home care, specialty physicians, and community programs.
May also help schedule medical appointments, accompany those in their care on trips, keeps track of important care-related medical and financial records.
Facilitates ongoing communications with the entire family so everyone is aware and understands the health of the person in their care. Care managers can also provide a placement assessment to an adult family home.

The Washington State Long-Term Care Ombudsman Program protects and promotes quality of life for people living in licensed, long-term adult care facilities (e.g. adult family home, assisted living facility, nursing home). An ombudsman: Advocates for the rights of clients in adult care facilities; Works with clients, families and facility staff to meet the needs and concerns of the people living there; and Provides a way to get complaints and concerns heard and resolved. The following people can use the Ombudsman Program: Residents living in a care facility and his/her relatives or friends; Administrators and staff of an adult family home, assisted living facility or nursing home.
