Managed Plan Member Services offered by Community Health Plan of Washington

Member Services Line -  Helps members locate primary care doctors and pharmacies in their area that are covered by their managed care plan. -  Helps members change to a different primary care doctor within the Community Health Plan of Washington network. -  Explains what services are covered under a member's plan. -  Lists what prescriptions are covered under a member's plan.   -  Offers information about disease management support and education services. -  Helps members file a grievance or an appeal against a doctor or decision made by the managed care plan.   -  Replaces a lost or stolen ID card.    Nurses Line   -  Answers health-related questions. -  Can help reach a doctor after-hours or set up an urgent care appointment.

Eligibility

Members of a Community Health Plan of Washington managed care plan. Washington state residents who are at or below 138% of the Federal Poverty Level may be eligible for this plan if they are not eligible for Medicare. Does not serve residents of the following counties: Clallam, Jefferson, Grays Harbor, Mason, Kitsap, Pacific, Wahkiakum, Cowlitz, Lewis, Thurston, Pierce, Okanogan, Chelan, Douglas, Grant. For those in Skamania, enrollment in managed care is not mandatory.
