Program of Assertive Community Treatment offered by Navos Mental Health & Wellness Center

Provides wraparound services to people living with mental illness. Works with clients or to maintain stability in the community and meet recovery goals. Teams consist of nurse practitioners, nurses, therapists, vocational counselors, peer support staff and SUD counselors who meet frequently with clients to provide treatment and support in the safety and comfort of their homes or in the community. Connects clients to educational opportunities where they can finish their GED or go back to college. Also provides employment support to help them find full- or part-time work. Offers individual therapy, case management, SUD services, medication management, and therapeutic groups.

Eligibility

Must be 18 years of age or older. "Priority" clients must have a diagnosis of a psychotic disorder and have been incarcerated or hospitalized twice in the last twelve months.
