Provides a patient-centered, family-oriented approach to care for anyone in the advance stages of a life-limiting illness. In Home Hospice care is provided wherever the patient calls home, which may be their own home, skilled nursing facility, adult family home or assisted living facility. The Hospice House is a General In-Patient care facility specifically geared toward caring for patients when their symptoms need professional management in an inpatient setting.
