Provides volunteers who can assist with door-thru-door transportation for low-income seniors, 55 and older, or adults with disabilities. Transportation is provided by screened and trained volunteer drivers in their personal vehicle. Rides must originate in Pierce County. Rides depend on volunteer availability. Does not qualify for services if: - Going to/from a Medicaid appointment (Medicaid Beneficiaries may receive services to appts not covered by Medicaid - Chiropractor, Eyeglass, pet care, etc; or possibly if Medicaid rides have been denied or are not covered) - Using a wheelchair (unless they fold and can be placed in the trunk and client can transfer themselves) - Living in a Group Home - Living in a Assisted Living Facility - Living in a Care Facility - Receiving COPES - Has a paid Caregiver - Client can use Transit, has own car, or lives with family member who has car. **Exception: Volunteer Services can fill in if family has one car and family member works full time which doesn't allow for client to receive rides** **Examples of Exceptions: client has temporary disability and cannot drive, or if client is at risk due to COVID, they can be considered “disabled” and not forced to take transit** Volunteer Services can provide rides for: COVID-19 Vaccines Medical Appointments Grocery Shopping Meal Sites Foodbank (Notate if clients are currently signed up and actively going to a particular food bank) Prescription Pick Up Dialysis – Limited Pet Care Appointments Senior Centers Bank Church YMCA Stores COVID-19 Vaccine: Volunteer Services provides rides to receive a Covid-19 Vaccine. Client must be going to an appointment at a vaccination site or a drive thru site. Volunteers drivers are able to wait in long lines or drive thru vaccination sites with the client. Volunteer Services will need the data and time of the appointment, the location, and if it is a drive thru or an appointment at a facility. Medical Appointment: Volunteer Services provides rides to medical appointments, including Dental, Chiropractic, Physical Therapy, Eyeglass, etc. Any appointment that is not Medicaid covered. *Possible exceptions to Medicaid Appointments: Client denied Paratransit/not receiving any other transportation. Will need explanation notated. Dialysis: Volunteer Services can serve as a supplemental transportation option for trips to and from dialysis. These trips will need prior notice to ensure driver availability. Medical Appointments involving Sedation: Volunteer Services can provide a ride to a client who has been under sedation and is accompanied by an attendant. The attendant must take on the liability for the client and be the one to accompany the client to the appointment. Drivers are not allowed to attend an appointment or procedure with a client. Picking up Prescriptions: Volunteer will take clients to pharmacies. Grocery Shopping and Food Banks: Volunteers will take clients to grocery stores and food banks. Volunteers may provide contactless grocery shopping/food bank pick up if requested, but not guaranteed. Dependent upon driver availability. (Notate if clients are currently signed up and actively going to a particular food bank). Pet Care Appointment: Animal in a carrier the whole time unless it’s a service animal. General Errands/Community Functions: Church, YMCA, Senior Centers, Meal Sites, Bank, Stores, Types of Ride Services EXPANDED due to Covid-19: Medical Appointments for Dialysis: Volunteer Services can serve as a supplemental transportation option for trips to and from dialysis. These trips will need prior notice to ensure driver availability. ***Everyone who qualifies for Volunteer Services is encouraged to apply in any case. Volunteer Services will be able to determine what they can do after receiving the intake from 211 and having a discussion with the client.

Call 211 to complete a transportation intake. Information needed for application includes: Client Name Address (Permanent address not necessary. As long as client can be at address agreed to. Notate client does not have permanent address) Phone Number Client Birthdate Client Gender Client Race Trip Purpose Household Income Client Income Monthly Rent Cost Out of Pocket Medical Expenses – can include insurance premium, Rx costs, equipment, etc. Type of Benefits Being Received Living Situation Martial Status Information on Disabilities and Health Conditions Name, Relationship, Gender, and DOB for everyone in the household Emergency Contact Once application is submitted, client will receive a call from Volunteer Services to provide approval/denial of transportation services. Volunteer Services will mail the client some documents to sign and return. Volunteer Services will inform client how to schedule rides.

Eligibility
Must be a senior, 55 and older, OR be 18 and older with a disability AND meet income guidelines. Disability Requirements: - Client must not be able to use regular transit. OR - Client may be eligible if they have a temporary disability. OR - Client must be able to walk on own or with walker/cane to vehicle, or have an attendant accompany to help. Volunteers cannot physically assist clients walking. - Volunteers can help stow assistive devices if client can stand and transfer into vehicle on own. Transportation Requirements: Must have no other form of transportation. Client MAY be eligible if they have a temporary disruption in transportation or has been denied rides. Client IS eligible if they are using Beyond the Borders. Household/Income Requirements: Definition of Household: Living alone or living with any family member: (Spouse, partner, child, parent, sibling, etc) Exception to household status if living with family: Client lives on same property but has separate power/electricity box. Each box counts as separate household. NOT included in Definition of Household: Roommates, renting room from landlord, shared housing, etc. (i.e., not family or partner). Income Eligibility Monthly Income Guidelines For Everyone in the Home, EXCLUDING Roommates: For individual or household: $4000.00/month maximum AFTER deductions If household is above 4000.00/month, household must meet 2023 Monthly Poverty Guidelines. (Household will be 9 people or more to exceed $4000.00/month) *Can deduct the total out-of-pocket medical expenses and total rent/mortgage.* Medical expenses can include but not limited to: Insurance premium, Prescriptions, Medical equipment/supplies, etc.
