Medical Care Financial Assistance offered at HealthWell Foundation

Provides financial assistance to eligible patients to cover certain out-of-pocket costs, including health insurance premiums, coinsurance and deductibles.   Also assists with prescription co-pays and other selected out-of-pocket health care costs.   Foundation allocates each patient a grant for full or partial assistance for up to 12 months.

Application process

Call to apply or for an application to be mailed.   If applying online, do print and submit the Statement of Treatment form and Income Statement. Assistance is granted on a first-come, first-served basis to the extent that funding is available.   Call or check the website to determine if your disease is on the list of diseases that the HealthWell Foundation supports. Some of these diseases are closed to new applicants.

Fee

None.   Provides full or partial assistance with eligible out-of-pocket health care expenses. The amount of assistance received depends upon income, insurance coverage, the amount of funding available for assistance and other factors.

Eligibility

Individuals with certain diseases, usually chronic or life-threatening, are eligible. Considers individual's financial, medical, and insurance situation.   Must have some form of health insurance. Individuals covered by private insurance, employer-sponsored insurance plans, Medicare or WA Apple Health (Medicaid) may be eligible.   Must make at or below 500% of the Federal Poverty Level. Medical criteria is based upon selected diseases.     Eligible diseases may include: - Age-related macular degeneration - Asthma - Cancer - Hodgkin's Disease - Non-Hodgkin's Lymphoma - Psoriasis - Rheumatoid Arthritis. Medication is only dispensed in the U. S.
