Provides direct financial support for pharmaceutical co-payments to insured patients, including Medicare Part D beneficiaries, who financially and medically qualify by having one of these diseases or conditions but not limited to: - Asthma - Bladder Cancer - Breast Cancer - Cancer Genetic and Genomic Testing - Cervical Cancer - Covid-19 - Chronic Pain - Electrolyte Imbalance - Hepatitis B - Hepatitis C - Hepatocellular Carcinoma - HIV, AIDS and Prevention - Homozygous Familial Hypercholesterolemia - Inherited or Acquired Lipodystrophy - Melanoma - Metastatic Breast Cancer - Metastatic Colorectal Cancer - Metastatic Gastric Cancer - Metastatic Prostate Cancer - Multiple Myeloma - Myelodysplastic Syndrome - Non-Small Cell Lung Cancers - Osteoporosis - Ovarian Cancer - Periodic Paralysis - Peripheral Vascular Disease - Prostate Cancer - Pulmonary Hypertension - Renal Cell Carcinoma - Rheumatoid Arthritis - Skin Cancer - Spinal Muscular Atrophy - Ulcerative Colitis - Uterine Cancers Once a patient is approved for the program they are granted an annual award up to a certain amount based on their diagnosis. Offers personal service to all patients through the use of call counselors. These counselors assist patients throughout the entire application process, screening for eligibility by collecting financial and medical information from everyone who calls to apply for the program. Offers assistance with insurance premiums through the COVID-19, Cystic Fibrosis, and HIV, Aids & Prevention funds.

Eligibility
- Patient must be currently insured and have coverage for the medication(s) for which they seek financial assistance - Patient must have a physician confirmed diagnosis of the disease under which they are requesting financial assistance - Patient must reside and receive treatment in the United States - Patient's income must fall below the income guidelines of the fund under which they are requesting financial assistance. All funds have income guidelines of either 300% or 400% or less of the Federal Poverty Guideline (FPG) with consideration for the Cost of Living Index (COLI) and the number in the household Patient will be informed immediately upon application if they qualify for assistance
