HOST (Homeless Outreach, Stabilization, Transition) Project offered at DESC's 216 Clinical Services
Provides critical survival services and engagement for adults who are experiencing severe and persistent behavioral health and co-occurring disorders in addition to homelessness. Operates as part of the King County-funded Prepaid Health Plan. Offers the following services: -Outreach and engagement services aimed at locating eligible individuals, building relationships, providing survival services and assessing individual needs. -Intensive case management and stabilization services with a full range of community support and treatment. -Development and support of maintaining connections to critical resources such as housing, psychiatric and physical health care, financial benefits and other ancillary support services. -Transition to long-term behavioral health or other appropriate ongoing services.

Application process
Initial contact is made with potential clients on the streets, at service centers, hospitals, etc. Key service sites receiving scheduled outreach include DESC, Angeline's Day Center, Women's Referral Center, Hammond House and Noel House. Referrals are accepted from service professionals, paraprofessionals and lay persons. Referral calls accepted.

Eligibility
Those experiencing homelessness in Seattle who are highly vulnerable, lack service connections or other support and who are unwilling or unable to engage in traditional service models. Cannot accept clients who could be or are served by traditional programs but don't have WA Apple Health (Medicaid). Clients have to be Medicaid-eligible eventually. Severe mental health condition, low functioning, lack of service connections, etc. are the primary screening criteria.

Provides four major types of services: comprehensive outpatient mental health and substance use disorder clinical and medical services; behavioral health crisis and outreach services; enhanced homeless shelters/emergency housing; and permanent supportive housing. Specializes in serving adults living with serious behavioral health conditions, co-occurring disorders and polysubstance use, and longtime experiences of homelessness and trauma.
