Homeless Medical Care offered by Neighborcare Health's Ballard Clinic

Provides a health care home for people experiencing homelessness or unstably housed adults as well as adults in supportive housing. Enrolled clients can additionally access behavioral health, social work and many more services to care for the whole person. Medical care includes health check-ups and screenings to help catch issues before they become serious.   Teams can also help take care of specific health conditions, such as high blood pressure, asthma or diabetes. Provides confidential screening, treatment and counseling for sexually transmitted infections (STIs), including HIV. Provides specialized services for veteran patients.   Refers to specialty care services. Family Planning & Women's Health Primary medical care providers, family medicine pregnancy care physicians, and midwives provide a number of reproductive health and women’s health care, including: -  Birth control counseling and prescriptions -  Regular gynecological exams -  Pregnancy testing -  Sexual health   Gender-affirming care Medical care providers can prescribe and help patients manage hormone replacement therapy (HRT) for female to male and male to female.   They can also make referrals to transition-related services including gender confirmation surgeries and non-surgical services such as hair removal.

Drop-in appointments are available 30 minutes prior to closing, but patients are encouraged to make an appointment whenever possible.  

Eligibility

Adults, 18 and older, who are experiencing homelessness or living without safe, stable and secure housing, and youth ages 12-26 experiencing homelessness or unstably housed. Also serves people who live in permanent supportive housing.

Fee

No cost to patients experiencing homelessness. Offers sliding scale fees. Accepts WA Apple Health (Medicaid), Medicare, and most health insurance plans. Insurance not required.

Neighborcare Health

Operates nearly 30 sites in Seattle, Vashon Island and Olympia that provide general medical care, dental care, integrated behavioral health, social services, insurance enrollment assistance, and midwifery care. Not all services offered at all locations. Three sites coordinate a drop-in homeless youth clinic. Another is a site of WIC. Additionally one site serves only homeless adult, and they have school-based health centers in 15 Seattle and Vashon schools. Apple Health/Medicaid, Medicare and most insurance plans accepted. Insurance is not required. Discounts available and no one is turned away if unable to pay.
