Molina Healthcare provides support services for individuals enrolled in Apple Health (Medicaid) managed care plans. Services include: · Locating in-network health care providers and pharmacies · Changing primary care providers (PCP) within the Molina network · Understanding covered services and prescription benefits · Managing chronic conditions through care coordination, case management, and educational resources · Help members file a grievance or an appeal if they are unhappy with a service or decision. · Replacing lost or stolen insurance cards

Call for more information and services. Molina members can also visit website to view personal health information, change primary care provider, get a new ID card, and more.

None.

Eligibility

Members of a Molina managed care plan. Washington state residents who are at or below 138% of the Federal Poverty Level may be eligible for this plan if they are not eligible for Medicare. Children in households of income under 300% of the Federal Poverty Level may also be eligible.
