Provides FREE Case Management for adults and assists with enrollment for Community First Choice (CFC), which offers in-home care, assistance with essential household tasks, adult family home care, assisted living, and congregate care. This is for adults who would require Medicaid services in a nursing facility. Services include the following: - In-home Personal Care: Provides assistance with activities, such as: housework, laundry, meal preparation, eating, shopping, bathing, dressing, transfer assistance, ambulation, self-medication, transportation and escort, and skin, hair and nail care. - Health Care Services: Care is provided by licensed health care professionals. Medical care must be prescribed by a physician. - Personal Emergency Response System: Offers emergency help to assist seniors and others with remaining independent in their own homes. By pressing a button worn as a pendant or wristband or connected to one's home phone, medical help can be summoned immediately. - Assistive Technology - Personal Care/Relief Care - Comprehensive Options Counseling for Long Term Care Planning - Family Caregiver Support
