Description
Offers case management for youth, 18-24 yrs of age who are homeless or at risk of becoming homeless. An intake will be conducted to assess the needs and barriers of the youth; they will then be referred to resources in the community that will help them overcome the barriers to reach their full potential.

Provides basic necessities, education support, employment support, housing support, behavioral health services, and a Coordinated Entry access point for youth, 13-24 years of age.
