A Public Health Nurse is available to assist with: - Connecting families to health care and other supportive services - Finding financial assistance for the child’s health related needs - Providing developmental screenings and referral as needed - Making sure that families have all they need when making the transition from hospital to home

M-F, 8am-noon, 1-5pm.
Application process
Call for an appointment with a nurse for an interview and assessment. Financial assistance requires a separate application, ask for details when calling. Referrals can be made by families, schools, medical providers, and any other person working with the family. Please call the CYSHCN coordinator or you can fax records/referrals to 509-524-2678.
Eligibility
Families that have a child between the ages of 0-18 years old that has or is at risk of having a chronic health or developmental condition that requires health and related services beyond what is typically required for child that age.

None. Services are free to all families, regardless of income
