Dementia Case Management offered by Magnolia
Offers ongoing, personalized support for individuals living with dementia and their caregivers. Clients are connected with a dedicated Care Navigator who will develop a customized care plan, help access tools and resources, and provide regular monthly check-ins. Care Navigators can meet with individuals and caregivers separately to ensure each person receives the tailored support they need. Services include disease education, caregiver training, emotional support, and practical guidance, all provided virtually. Complements client's existing healthcare team and is appropriate for those just starting to explore questions about memory loss as well as those navigating later-stage or end-of-life dementia care.

Call or complete online form. Scheduler will explain the services, help determine the best fit. Explains how services are billed as well as insurance coverage.
Fee
For those with Medicare as their primary payor, dementia care management support is at no cost. In network with the following providers: Regence (Blue Cross Blue Shield), Aetna, United Healthcare, Anthem, Humana, Medicare, as well as most Medicare Advantage Plans. A co-pay or deductible may apply.

Individuals in Washington state experiencing memory loss or other serious condition related to aging as well as their caregivers.
