Offers a long-term daily program for adults who have an acquired brain injury. Develops skills for independence and enhances meaningful daily life. Program components include: - Physical rehabilitation - Health services - Cognitive skill building - Psychosocial skills building Activities may include: - Music - Strength, endurance, range of motion, and motor skills groups - Chronic Disease Self Management - S. A. I. L evidence-based exercise program - Expressive art groups

Eligibility
Clients ages 18-59 must have an acquired brain injury and wish to be actively involved in programming choices. Program focuses on serving individuals who desire to enrich their daily life using current and developed skills and who meet DSHS criteria for adult day health or are eligible for funding through Respite, Dementia Day, VA or ACL.

Acceptable funding sources include: - DSHS/HCS long term care programs including COPES, MAC & TSOA, etc. - Veterans Administration - Long term care insurance - Private pay - Respite - Memory Care & Wellness Services - ACL Grant Contact intake coordinator for more information.

Services include: care consultation, adult day health, mental health case management and ElderFriends Volunteer Companionship. Adult day health program for people with developmental disabilities focuses on recreation activities.
