Funded Care offered by Muscular Dystrophy Association
Funds care for ALS patients at the ALS Center at UW and neuromuscular diseases at the MDA Clinic at UW and the MDA Clinic at Seattle Children's Hospital, where patients can get diagnostic and follow up care visits with experienced specialists as well as get swallowing assessments and physical and occupational therapy.

Call for information. Physician referral is preferred.
Eligibility
Patient must have diagnosed or suspected one of the 43 neuromuscular diseases MDA covers, including ALS.
Fee
Patient insurance is billed by UW Medical Center for services. MDA grant assists with remaining patient responsibility for specific list of outpatient services provided by doctors associated UW Medical Center.
