Provides services to individuals needing assistance with guardianship, trust and power of attorney matters. Acts as legal guardian of person and/or estate, trustee or attorney-in-fact for individuals with cognitive disabilities or who are frail/elderly or severely injured. Information and Referral: - Advises families and relatives of individuals with disabilities on services related to guardianships, trusts and powers of attorney.

Call for phone screening and client intake/application form. Potential clients are screened by an intake review panel.

Eligibility
Applicant must have a need for services due to cognitive incapacity or severe injury. Must demonstrate inability to manage either their personal or financial affairs and be at risk for abuse, neglect or exploitation. To be accepted for guardianship, trust or financial services, the candidate must have income or resources that will allow collection of fees for services.
