Vital Statistics offered by Public Health in Seattle & King County

Issues copies of birth and death certificates for anyone born in the state of Washington since 1907 and death certificates for deaths that occurred in King County since 1907. Can issue death certificates for all deaths that occurred in the state of Washington since 2015. Application process

Order in person in the lobby; most orders are fulfilled in about 30 minutes. Other orders, (mail, fax, phone or online) are typically fulfilled within 7-10 business days. Order forms for birth and death certificates are available in English and Spanish.

Eligibility

To purchase a birth certificate, you must be a qualified applicant. Qualified applicants are: self, spouse or domestic partner, parent, stepparent, legal guardian, child, stepchild, sibling, grandparent, great grandparent, legal representative, authorized representative, or government agency or the courts if the birth certificate is used for official duties. If you are not one of the people listed above, you will not receive a birth certificate.

Fee

Fees for services.   -  Birth certificates: $25. - Death certificates: $25.   -  Payment with credit card costs $15. 50 extra.   -  Orders placed by mail with payment by check or money order cost $4 extra. Once orders have been placed no cancellations or refunds per the Revised Code of Washington (RCW 70. 58a)
