Offers several different programs and supports that help people take control of their health, manage long-term conditions or health risks, and plan for long-term care. Individual Health Coaching: FREE service. Coaching can help with: - Tobacco Liberation, Offers Diabetes Wellness Services for those with Diabetes of Pre-Diabetes. Support Groups: - Pre-Diabetes/Diabetes Self-Management Support Group: Meets 1st and 3rd Tuesday at Morton General Hospital, Room 2. - Bereavement Support: Meets 2nd Wednesday of the month, 10am-11:30am at Morton General hospital, Conference Room 2. - Better Breathers Club (Lung Disease): Meets 2nd Thursday of the month, 11am. Free, individual appointments for Consultation and Assistance Related to Long Term Care Planning Provides assistance to people who are having to plan and make difficult decisions related to long-term care or end-of-life care. Assistance can include: - assisting with applications for Medicaid, - support for palliative and end of life care, - financial review related to long term care payment options, - information on advance directives, - assistance related to long term care planning.
