Provides birth and death certificates.   Also issues Death Certificates and permits for burial and cremation, including permits for removal and transfer of remains to other counties, states, or countries.

Visit the Health Department website and download an application or contact the Vital Records office and request an application.   It is also possible to order death and birth certificates online at www.vitalchek.com.

No restrictions.

Fee

Birth and death certificates are both $21 each. Shipping costs vary.

Offers public health services for individuals, communities, and businesses, including: - water, restaurant, waste disposal, and septic system inspection; - monitoring and responding to outbreaks of communicable diseases, including TB, Hepatitis, HIV/AIDS, sexually transmitted diseases, and others; - substance abuse counseling, - health counseling for prenatal clients and parents of children in the home; - health education and general health information and referral.
