Options Counseling offered by Sound Generations
Provides guidance to individuals to help them make informed choices about supports and services in order to live independently in the community. Focuses on those who need additional support around a transition or change in their lives such as facing a new health condition, recent move, or change in social network. Specialists can meet one-on-one. Works with the individual to explore options that can help them live independently longer in a manner that is safe and meaningful to them. An options counselor can meet with the older adult one on one to: - Develop a personalized action plan - Assess and refer to additional resources - Provide limited assistance in filling out paperwork - Can provide limited phone-based advocacy to connect client to resources During visit CANNOT provide: - Housework - Errand running - Personal care services - Transportation - Legal representation or advice

Application process
Call to make a referral. Accepts referrals from Community Living Connections specialists and those calling on client's behalf. CLC specialist must allow clients the option of calling themselves and may not call on behalf of a 3rd party. CLC specialists may only call on behalf of a client for whom they have obtained direct consent. If no answer, leave a voicemail with as much information about client as possible including: - First and last name - Phone number - Address - Date of birth - Reasons Options Counseling may be needed - Description of services needed Third parties can make a referral by calling and leaving the client's information. Sound Generations will contact the client within one week, however can follow up sooner if considered urgent.

Eligibility
Adults ages 60 or older and adults ages 18 or older with a disability. Those that need guidance on making care choices, but are capable of connecting to provided services on their own. Must live in King County. Must not currently be receiving case management through Home and Community Services or a housing authority.

Operates a network of senior centers. Manages meals and grocery delivery programs. Offers minor home repair services including making disability modifications. Arranges for transportation. Provides information on legal topics including end-of-life planning and coordinates an elder law clinic. Offers health and wellness programs.
