Health Care Advocacy & Education offered by YWCA Seattle King Snohomish's Health Care Access
Meets with low-income individuals and families over the phone, virtually, at day centers, and other locations throughout King and South Snohomish counties to provide support and education on how to access health services and navigate systems of care. Community Health Worker/Navigators provide help with Apple Health (Medicaid) enrollment, troubleshooting, referrals to health clinics or other services, problem-solving to help access care, basic health education, and more.

Eligibility
Any individual/family who is unstably housed, unhoused or at-risk of losing housing, lives in emergency shelter, uses day center services, couch surfing, doubled-up with family/friends, or lives in permanent supportive housing within King or South Snohomish counties.
