Provides birth and death certificates in Washington State; only provides birth certificates for individuals born in Washington since 1907. Birth and death certificate applications can be downloaded (English) or (Spanish) from website. Death certificate are available approximately one week after event. It would be best to contact agency directly to make sure the document is still on file locally before ordering.

The cost for each certificate is $25. Acceptable forms of payment are cash, money order, or credit card (there is a $2.50 or 3% fee for using a credit card whichever is greater).
